Activities and Athletics Registration Information
Minnesota State High School League (MSHSL) Eligibility and ISD709 Policies/Regulations Online Form
Activities and Athletics registration is to be completed online.
Your child will not be allowed to participate without this form completed.
Online Registration is available to currently enrolled students only.
2024-2025 activities/athletics registration
MSHSL Physical Form
Every student athlete is required to have a valid sports physical on file with the East Athletic Office prior to any participation in any sport. Make sure your student's physical is up to date. They are typically valid for 3 years unless your student's physician states differently. You must print this out and bring it to your physician's office to be filled out. There are copies in the Athletic Office if you do not have access to a printer.
NO PHYSICAL = NO PLAY!
Your students' physical status (expiration date) can be found in the Parent Infinite Campus Portal. Menu (three lines on the top left of your screen) then select ‘MORE’ then ‘ACTIVITY DEPARTMENT’

Travel Release Form
If your student is not riding on the district provided transportation either to or from an event, this form needs to be filled out and brought to the athletic offices at East High School prior to the event.
Return to Play Form
This form must be filled out and returned to the athletic offices at East High School before a student may resume participating in a sport following an injury (concussion, etc.)
East Activity Payment Options
Reduced participation fees for those that qualify
Families who qualify for free or reduced lunch must fill out the required forms either by printing the form below OR through the infinite campus portal. (In the IC App, Click Menu, More, Meal Benefits, Meal Benefits Application)
2024-2025 Application for Benefits
No payments are required until after team selections have been made, at that time payments can be made in student's infinite campus account under payments OR cash / check can be turned into the athletic offices (please make sure to include you students first & last name along with activity they are participating for). If you choose to pay via CC, this must be done through Infinite Campus and there is 4% convenience fee.

Make Payable to East High School
Include your student's first name, last name, and activity you are paying for on your check
Have your student drop payment off at the activitiies office

- All participation fees must be paid prior to the first scheduled event.
- The participation fee is reduced to students who qualify for reduced lunch to $25.00 for each activity.
- The participation fee is waived for students who qualify for free lunch.
Family Limits $1,000
Participation Fee Refunds: Students who choose to participate in athletics must realize there are no guarantees regarding playing time or injuries. No participation fees will be refunded after the day prior to the first scheduled event.

Transfer and Exchange Students
Transfer or Exchange Students - Contact the AD by either stopping in to the office or via email at shawn.roed@isd709.org
Transfers, exchange students, or Woodland Hills transfers will not be able to practice or participate until they are cleared for play by the Duluth East Activities Office and the MSHSL.
